How do I decide between Medigap and Medicare Advantage?

A decision worksheet rather than a sales pitch. Six questions that settle it for most people.

This is the decision everything else hangs on, and it is usually made in a hurry with incomplete information. Here is a way to work through it deliberately.

Question 1: where are your doctors

List your primary care physician, every specialist you see, and the hospital you would want to use. Check each against the Advantage plans available at your address.

Question 2: how do you actually live

Yes to any of these leans strongly toward a supplement, which works nationwide with no service area. See Medigap and travel.

Question 3: what is your health outlook

Not how you feel today. What you know about your conditions and your family history.

This matters because of the underwriting clock. If a supplement is likely to be valuable to you later, the only guaranteed chance to buy one is now.

This is the asymmetry that should shape the whole decision. Advantage plans can never turn you down, at 65 or at 85. Supplements can, outside your six month window. So the door that closes is the supplement door, and it closes quietly.

Question 4: run the two totals

  1. Supplement path: twelve months of supplement premium, plus twelve months of a Part D plan, plus the annual Part B deductible. This is roughly both your best and worst case.
  2. Advantage path: twelve months of plan premium, plus the annual out of pocket maximum. That is your worst case. Your best case in a healthy year is close to nothing.

Which worst case would you rather face, and what would you pay in good years to avoid the other one?

Question 5: what are the extras worth to you

Convert dental, vision, hearing and fitness benefits into dollars you would actually spend. If you have not seen a dentist in years, a cleaning benefit is worth little. If you need significant dental work, it may be worth real money.

Then check what a standalone dental and vision policy would cost alongside a supplement, which is often less than people assume.

Question 6: how do you want to deal with it

If it is still close

When the numbers are near even, the tiebreaker is usually the underwriting clock. Starting on the supplement side keeps every future option open, since moving to Advantage later is always available. Starting on Advantage may close the supplement option permanently.

That is not an argument that Advantage is wrong. Most Medicare beneficiaries in this county are on Advantage plans and many are well served. It is an argument for making the choice on purpose. See the side by side comparison.

Common follow-up questions

Which one do most people here choose?

Advantage, by a wide margin. Hidalgo County has high Advantage participation and deep local networks, which is a legitimate reason it works well for many people.

Can I change my mind later?

You can always move from a supplement to Advantage. Moving the other direction may require passing underwriting, which is the asymmetry to plan around.

Is one of them objectively better?

No. They solve the same problem with opposite trade offs. The right answer depends on your doctors, your travel, your health outlook and your tolerance for surprise bills.
