What is Extra Help and do I qualify?
A federal subsidy that cuts prescription costs sharply for people with limited income. Widely available in South Texas and widely unclaimed.
Extra Help, formally the Low Income Subsidy, reduces what you pay for Medicare prescription coverage. For people who qualify it is one of the most valuable benefits in the entire program, and a lot of people who would qualify never apply.
What it does
- Reduces or eliminates your Part D monthly premium
- Reduces or eliminates your annual deductible
- Caps what you pay per prescription at low fixed amounts
- Waives the Part D late enrollment penalty entirely
- Gives you additional opportunities to change plans during the year rather than waiting for October
That penalty waiver is worth calling out. If you have been avoiding Part D because of an accumulated penalty, qualifying for Extra Help removes it.
Who qualifies
Eligibility is based on income and on resources, meaning savings and investments. Your home, your vehicle and your personal belongings do not count as resources.
The limits change annually and they are higher than most people assume. This is the main reason for underclaiming: people rule themselves out without checking.
If you are anywhere near the range, apply. There is no cost, no penalty for being denied, and the application is short. Being told no costs you nothing. Never applying can cost you thousands.
Automatic qualification
You get Extra Help automatically, without applying, if you have any of these.
- Full Medicaid coverage
- A Medicare Savings Program such as QMB, SLMB or QI
- Supplemental Security Income

How to apply
- Apply through Social Security at ssa.gov, by phone, or at the local office. The application is short.
- Separately, apply for a Medicare Savings Program through Texas Health and Human Services at yourtexasbenefits.com. Different agency, different application.
- Have your income information and a rough figure for your savings and investments ready.
- If denied, ask why. Sometimes it is a documentation issue rather than a genuine ineligibility.
After you are approved
You keep your existing plan or choose a new one, and the subsidy applies to it. Some plans are better suited to Extra Help recipients than others, so it is worth reviewing your plan once approved rather than assuming your current one is optimal.
You also get more flexibility to change plans through the year, which most people are never told.
Why this matters locally
Hidalgo County has a significant population who would qualify for Extra Help, a Medicare Savings Program, or both. Every month someone goes unenrolled is money left on the table, often from a Social Security check that is already stretched.

No. Your primary home, your vehicle and personal belongings are not counted as resources for Extra Help.
